Consumer-Directed Community Supports (CDCS) is a unique service option that gives persons more flexibility and responsibility for directing their services and supports, including hiring and managing direct care staff. CDCS may include services, support and/or items currently available through the MA waivers, as well as additional allowable services that provide needed support to persons.

If you are interested in Consumer Directed Community Support (CDCS), please contact your current case manager. People who would like to use CDCS services must be currently open to a waiver or the Alternative Care program and must complete a file clearance process.

New 2011 CDCS Renewal Packet:

  • The Community Support Plan for CDCS has been streamlined and now eliminates requirement to complete and submit worksheets. Similar information that we would need to approve the plan should still be included within the plan.
  • The revised community support plan now includes the updated signature page for the CSP, Year End Results form within the plan, and space for including contacts for doctors, therapists, and other team members.
  • Please complete the updated CSP as well as the forms included below for your renewal.

Please complete and return the following:

  • Notice of Privacy Practices
  • Health and Safety Signature Page (unless you are making major changes-then include the complete plan).

Support Planner (CDCS) Monthly Meeting

All certified support planners who are providing services in Hennepin County are encouraged and welcome to attend a monthly meeting to obtain informational updates. We address your questions about policy implementation and plan completion. All registered support planners receive the meeting agenda by e-mail prior to the meeting.
